Abstract

Reducing hospital readmissions is a critical goal for healthcare systems worldwide, as high rates of readmission not only impact patient outcomes but also contribute significantly to healthcare costs. Nursing interventions play a crucial role in preventing unnecessary readmissions by providing comprehensive care and support to patients during their hospital stay and after discharge. This review article aims to assess the effectiveness of various nursing interventions in reducing hospital readmissions. The review will begin by examining the current landscape of hospital readmissions and the factors contributing to this phenomenon. It will then explore the different types of nursing interventions that have been implemented to address readmission rates, including patient education, medication management, care coordination, and transitional care programs. The effectiveness of these interventions will be evaluated based on existing literature and studies that have investigated their impact on readmission rates and patient outcomes. Furthermore, the review will analyze the challenges and barriers faced by nurses in implementing these interventions successfully, such as limited resources, time constraints, and communication issues.
